SocaCize PAR'Q & Waiver

Before participating in your FIRST SocaCize class, a Par'Q form and waiver must be completed.

You only have to complete this once, unless it has been more than 60 days since your last SocaCize class. 

If these forms are not completed, your will not be allowed into class.
